I, the undersigned, in consideration for my voluntary participation in Valley United SC organized soccer, do hereby willfully acknowledge that my signature below attests to my understanding and agreement that:My player status will be kept in good standing. I will not compromise myself in such a way as to do harm to Valley United SC, the league or tournament, knowing that players may be dismissed from participation, with possible loss of payment or dues,

for violent conduct or unsportsmanlike behavior on or off the field of play. I agree to pay for any and all damages to any property or indemnities caused by me willfully, negligently, or otherwise.

Soccer is a physical, contact, sport that involves the risk of injury. I assume all risks and hazards associated with my participation in the sport. I am in proper physical condition to participate in soccer practices and games and have no illness, disease or existing injury or physical defect that would be aggravated by my participation. I will inform my coach if this status changes. I further acknowledge that this risk may involve loss or damage to me or my property, all illness, disability, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care. I have a current medical consent form in force. I will wear shinguards, properly-fitted and appropriate shoes, and other protective equipment (e.g., mouth-pieces), as provided by soccer rules, to all events.

I am well aware that the participation of participant may have possible exposure to and illness from infectious diseases including but not limited to influenza, COVID-19, MRSA. I acknowledge that there is rules in place to reduce the spread of the illness but the risk of serious illness and death does exist including participant and anyone they come into contact with.

Valley United SC and the league does not have personal injury insurance that covers my participation. Therefore, I should have a current, active, personal injury insurance that covers my participation. Therefore, I should have a current, active, personal injury insurance policy in force, which covers my participation. Under any condition, I am responsible for any and all medical expenses arising from my participation, games. I have the right and responsibility to inspect the equipment and facilities prior to events and, if i believe that anything may be unsafe, I will advise the coach or supervisor of the condition and may refuse to participate. Participation assumes consent.

I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, Valley Soccer League Inc., the team and facility in use, their associated directors, administrators, officers, managers, employees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and my participation in, thier soccer programs.

I have completely read this document and fully understand its contents, I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. By checking “Agree to the terms“, I attest to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns.

COVID-19 Informed Consent, Acknowledgement of Risk, and Waiver & Release of Liability – Youth Sports Organizations

Information About COVID-19 Risks of Participation

The current pandemic health emergency is related to the highly contagious novel coronavirus (“COVID-19”). COVID- 19 is an illness caused by a virus that can spread from person to person, primarily through respiratory droplets. The impact of COVID-19 on the health of the public is not yet fully known. COVID-19 is a new disease and there are limited data and information about the impact of many underlying medical conditions on the risk for severe illness from COVID-19. Severe illness from COVID-19 is defined as hospitalization, admission to the intensive care unit (ICU), intubation or mechanical ventilation, or death. Among adults, the risk for severe illness from COVID-19 increases with age and certain pre-existing health conditions, with older adults at highest risk. While fewer children have been sick with COVID-19 compared to adults, children can be infected with the virus that causes COVID-19, can get sick from COVID-19, and can spread the virus that causes COVID-19 to others.

While there are now authorized and recommended vaccines to prevent COVID-19 in the United States, there is currently a limited supply of these vaccines. In addition, multiple variants of the virus that causes COVID-19 have been documented in the United States and globally during this pandemic. Therefore, the best way to prevent illness is to avoid being exposed to the virus that causes COVID-19.

Although the Conejo Recreation & Park District (herein referred to as “CRPD”) has implemented certain preventative measures consistent with applicable rules, regulations, federal and state orders and guidance, and guidance from public health officials related to COVID-19, CRPD cannot ensure that participants and/or their families or others in the participant’s household will not become infected with COVID-19. Moreover, CRPD cannot protect against exposure to or infection by COVID-19 that occurs due to the actions, omissions, and/or negligence of participants or others, including CRPD staff members.

California Public Health Guidance for Organized Sports and Recreation

COVID-19 continues to pose a severe risk to communities and requires all people in California to follow recommended precautions. The California Department of Public Health (“CDPH”) issued revised public health guidance for youth and adult sports on February 19, 2021. The guidance is intended to provide direction on all organized youth and recreational adult sports activities to support a safe environment for these sports. A copy of the most recent version of these materials is enclosed for your information.

Additionally, the guidance applies to all organized youth sports and recreation— including school- and community- sponsored programs, and privately-organized clubs and leagues — and adult recreational sports. The guidance may be subject to change as new information becomes available. You are encouraged to regularly consult updated health guidance information as well as specific guidance for sports and recreation including the following websites:

COVID-19 Informed Consent, Acknowledgement of Risk, and Waiver & Release of Liability

Before participants may participate in the below-referenced activity, the Conejo Recreation & Park District (herein referred to as “CRPD”) requires that the youth sports organization duly authorized representative listed below read the information in this Notice, the enclosed materials, and sign below to ensure they are informed of and understand the risks related to COVID-19 that are associated with participation in sports and recreational activities.

In signing the below, you attest that you have read the Notice and understand the risks related COVID-19 that are associated with participation in the below-referenced activity. You further understand that there are cardiovascular and other health and safety risks associated with returning to athletic activities after COVID-19 infection. You also acknowledge that the health and safety risks posed by COVID-19 cannot be eliminated, despite the implementation of reasonable and age-appropriate precautions and protocols. You further understand that because of the COVID-19 pandemic and the risk of transmission inherent in sports participation, the CDPH has categorized youth and adult sports into four tiers based upon their level of contact and transmission risk. You also acknowledge that there may be risks associated with adhering to certain mitigation strategies recommended and/or required by the CDPH such as wearing a face covering mask that completely covers the nose and mouth, including during active play. Finally, given the unknown nature of COVID-19, you understand that it is not possible to list each and every specific risk associated with COVID-19 and that neither CRPD nor public health officials can guarantee that any participant will not come into contact with someone infected by COVID-19 and/or contract such illness.

I understand that the choice to participate in the below-referenced activity is voluntary. By opting to have participants of the sports organization listed below participate, I agree the below named youth sports organization and its affiliated teams and participants will abide by all applicable public health and safety protocols set forth by the California Department of Public Health, the County of Ventura as well as any other rules CRPD may require. I understand that promoting public health is a shared responsibility and that every member of the community must do his/her part to minimize risks. I also understand that on behalf of my affiliated youth sports organization, a similar informed consent form must be distributed to and signed by the Parent/Guardian of every participant, prior to participating in competition activities.

In consideration for being permitted by CRPD to utilize its sports facilities, and participate in the below-referenced activity, the youth sports organization listed below fully ASSUMES ALL RISKS, inherent and otherwise, whether or not described above, in connection with participation in the activity and hereby waive, release, and discharge any and all claims for damages for injury, harm, or illness including, but not limited to contracting COVID-19, which may in any way relate to participation in said activity. This release is intended to discharge in advance CRPD (including its officers, employees, volunteers, and agents) from any and all liability arising out of or connected in any way with participation in said activity, even though that liability may arise out of active or passive negligence or carelessness on the part of the persons or entities mentioned above.

I HAVE RECEIVED AND CAREFULLY READ THE COVID-19 RETURN TO COMPETITION PROTOCOLS AND THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH OUTDOOR AND INDOOR YOUTH AND RECREATIONAL SPORTS GUIDANCE, RELEASED FEBRUARY 19, 2021.

I HAVE CAREFULLY READ AND UNDERSTAND THE NOTICE ABOVE (INCLUDING THE INFORMATION ABOUT COVID-19 AND THE RISKS OF PARTICIPATION) AND HEREBY KNOWINGLY AND WILLING CONSENT TO THE PARTICIPATION IN THE BELOW-REFERENCED SPORT/ACTIVITY.
